Abstract

Elderly patients with hip fractures are usually frail and often present with osteoporosis and comorbidities. Therefore, operative methods that can overcome these difficulties and can facilitate recovery to the pre-fracture status should be selected. Internal fixation is generally less invasive, however, shows higher reoperation rate than arthroplasty. Arthroplasty, resecting fracture site, does not need to achieve bone union and shows lower reoperation rate. However, arthroplasty is more invasive and shows higher postoperative complication rate than internal fixation. For displaced femoral neck fractures in elderly patients, arthroplasty is recommended, while internal fixation is usually recommended for nondisplaced one. For intertrochanteric fractures, internal fixation, especially intramedullary nailing is generally performed.
